Medicare Type 2 Diabetes
GROUP EXERCISE PROGRAM
Medicare Type 2 Diabetes Group exercise program The Program is designed to assist patients diagnosed with type 2 diabetes to self-manage their condition, reduce the severity of the disease and reduce the risk of long-term complication. It consists of an individual initial assessment and 8 structured group exercise sessions that contain lifestyle modification interventions. Patients referred to the program with a Medicare Type 2 Diabetes referral will be bulk billed for the services. These group sessions are in addition to the 5 individual allied health services available under Medicare.
